Essential Duties and Major responsibilities:

  • Organization & coordination of Suboxone and Vivitrol schedules; conducting intakes, high risk nurse visits, and any additional visits applicable to care plan of patient
  • Be the patient contact and point person for accurate information for patients and providers regarding protocols for Suboxone and Vivitrol
  • Process referrals to behavorial health and/or communicate with other departments when needed to relay pertinent information
  • Provide support during screening, intake, induction, stabilization & maintenance phases of patient treatment.
  • Conduct random nurse visits / pill counts as directed by provider to insure compliance and minimize diversion of prescribed medication
  • Responsible for providing clear vision and oversight for all aspects of the program and any grants associated with the program
  • Required to attend mandatory program meetings and trainings
  • Develop and maintain relationship with DPH, including monthly submission of data management system file extractions
  • Develop individualized care plan in coordination with the patient and provider to reach timely and appropriate diagnosis
  • Tracking, follow-up and reporting of all diagnostic services, diagnostic test results, referrals for diagnostic tests, tracking of results, and closing the loop for all test
  • Communication with providers, patients and the patient’s families when appropriate
  • Plan, coordinate, monitor, evaluate and document progress toward meeting goals and objectives on the case management plan
  • Maintains the treatment database with health information for those patients enrolled in the program and pertinent to any associated grants
  • Demonstrates excellent customer service and effective communication skills by email or calling via telephone in regards to meetings, staff updates, and other correspondence as needed
  • Demonstrates team building skills
  • Demonstrates analytic and data management skills using a variety of PC based software, including MS Office (Word, Excel, and Power Point)
  • Demonstrates understanding and commitment to the mission of the health center
  • Demonstrates understanding and commitment to the established CHC Values and Standards
  • Works independently on special nonrecurring and ongoing projects as Perform related duties as required and assigned.

Minimum Qualifications:

  • Associates Degree in Nursing with active license as a RN; BSN preferred
  • 2-3 years experience with MAT
  • AHA BLS Health Care Provider certification or equivalent
  • Communication skills as evidenced by preparation of oral and written materials including presentation, reports, memoranda and correspondence of both a technical and confidential
  • Analytical ability as evidenced by developing solutions and recommendations with guidance from superiors in grant and contract




Associates or better in Nursing.


Bachelors or better in Nursing.
