The Early Intervention (EI) team provides strength-based, family-centered services for young children under three years of age who have developmental delays or are at risk of a developmental delay. Services are designed to meet the needs of each infant or toddler, and the needs of their family. The EI team provides services in the home/community setting with the goal of enhancing the child’s physical, cognitive, communication, social-emotional and adaptive skills.
Responsibilities:
1.Provides direct intervention services to children 0 - 3 and their families.
2. Carries out service coordination functions when appropriate as defined by the Early Intervention standards
3. Assists in transition planning prior to discharge from the program.
4. Referrals to outside agencies as needed.
5. Maintains client records and other administrative duties according to DPH and Lipton Early Intervention policies.
6. Adhere and complete all CHL's protocols, policies, and procedures.
7. Early Intervention services are provided both in-person and/or by Telehealth.
8. Attends in-service education and professional meetings as required.
9. Respects diverse views, approaches, and contributes to maintaining an environment of professionalism, tolerance, civility, and acceptance towards all employees, clients, families, and visitors.
10. Maintains regular, reliable, and predictable attendance.
11. Ability to work independently and contribute to the overall success of the team.
12. Flexibility, excellent interpersonal skills, verbal, written and communication skills.
13. Maintains productivity standards according to the collective bargaining agreement.
14. Perform other related duties.
Qualifications:
Bachelor's Degree or higher required
CPR/First Aid certification or obtain within 6 months of employment.
Department of Public Health CEIS certification or eligible for certification.
NPI or apply for NPI (National Provider Identifier Number).
